We would like to thank you for choosing Park Place Spa and Body Sculpt. As Fort Lauderdale’s premier luxury medspa. Our primary mission is to deliver the best and most comprehensive aesthetic products and services available. As one of our clients we would like to keep you informed of the current office and financial policies. Please read each of the following sections carefully:

PAYMENTS: I fully understand and agree that I am financially responsible for the full cost of the agreed upon products and/or services. ALL PAYMENT IS EXPECTED AT THE TIME OF PURCHASE. Park Place Spa and Body Sculpt only accepts payment in the form of Cash, VISA, MasterCard, American Express, Discover, and Cherry Financing.

INSURANCE: Park Place Spa and Body Sculpt does NOT participate with or submit billing to any private insurance companies. I fully authorize Park Place Spa and Body Sculpt to charge my credit card on file for any fees associated with missed or late, rejected, or bounced payments.

SERVICES AGREEMENT: I understand Park Place Spa and Body Sculpt has the right to refuse service and/or dismiss a client from any service or product at any time. I also understand that I may not be a candidate for certain medical services, and it is at the full discretion of the medical provider to determine whether I am a candidate for any service provided.

NO RESULT IS GUARANTEED: I clearly understand that my payment is for the product(s) and/or treatment(s) purchased during the term of the agreement and not for any specific result. I understand that results will vary and include the possibility of no results, and that additional treatment(s) and/or product(s) may be obtained at an additional cost to me.

CANCELATION POLICY: Life happens! We understand that a situation may arise that could force you to cancel or postpone your treatment. Please understand, however, that your treatment time is reserved exclusively for you, meaning other patients are not offered this date and time in anticipation of your visit. Changes to your appointment affect your provider and other clients. Therefore, should you find it necessary to cancel your scheduled appointment, please do so within 24 hours to avoid a cancellation fee of your deposit.

REFUND POLICY: ALL SALES ARE FINAL. Before a service is performed or a product is purchased, please consider all the required protocols and side effects. We are committed to patient satisfaction and are available to answer any questions or concerns you may have regarding the services or products we offer before purchase.
